FISCAL ANALYSIS
ASSUMPTION
Officials from the Department of Revenue (DOR) state their agency will need to:
► Revise related policies and procedures as well as the DOR website to reflect the changes.
► Create the new form that is to be used in place of the physician's statement.
DOR will need to revise the disabled person plate or placard renewal form to notify potential applicants who need a physician's statement of the change that in lieu of the physician's statement the notarized form provided by the director in the case of religious affiliation would need to be submitted. Costs for the change would be minimal if any.
DOR assumes that the form to be presented that is required by this legislation will be available on the internet so no cost is being shown.
